Apply to be a Patient Ambassador
UCSF Benioff Children's Hospitals select Patient Ambassadors each year to help build community awareness and raise critical funds for our patients and their families.
Eligibility Requirements
-
Must be a current or former patient of UCSF Benioff Children’s Hospitals
-
Family must be willing to speak about the medical journey of the patient ambassador, if the patient can’t speak on their own
-
Must be able to speak on behalf of UCSF
-
Must be willing to sign a media release form
-
While we would like to incorporate everyone into this program, we will select a limited number of Patient Ambassadors to represent us each year
What is Required?
We ask that every Patient Ambassador share their medical journey with the UCSF Benioff Children’s Hospitals Foundation team. As an ambassador, you will also participate in a photoshoot and receive UCSF branded apparel. We may also ask you to share your story in a video or with the local media.
If interested, you can participate in community events held throughout the year. There will be opportunities to be a “champion” for different corporate partners, attend community events, speak at fundraising kickoff events and so much more.
Every Patient Ambassador will need to have a signed consent and release form, which we will email after the application is completed.
Interested?
If this sounds like an opportunity you and your child would be interested in, please fill out the application form below and a foundation representative will be in touch shortly.
